Description

Lincoln County's Primary Care clinics, Coastal Healthcare, are looking for a Community Health Licensed Practical Nurse (CHLPN) to join their team. The person in this role will provide direct patient care supporting Provider Care Teams in the Primary Care clinics. Assist the Provider in performance of procedures including wound care, immunizations, and medication administration, point of care testing, telephone prescription orders/refills, medication prior authorizations, maintaining patient Electronic Health Record (EHR) and communication of test results. Provide safe and quality nursing care under the direction of the Provider or Community Health RN within LPN scope of practice. This role is also responsible for promoting health through education and care coordination to optimize patient health outcomes.
$28.29 - $30.01 per hour*

EDUCATION
Completion of a Licensed Practical Nursing certificate program.
EXPERIENCE
Two-Three years LPN experience.
Substitution Note: Practicums in Nursing programs may substitute years of experience.Driver’s License
Oregon Driver's License required within 30 days of hire.
Other Licenses
Must possess a valid license to practice in the State of Oregon as Licensed Practical Nurse as defined by statutes or administrative rules, which include but are not limited to ORS 678.010 to 678.360 at the time of appointment. The employee is required to maintain current and active licensures and certifications, as applicable, and to provide the appropriate verifications at the time of renewals.
