Community Health Licensed Practical Nurse - Coastal Healthcare

Lincoln County's Primary Care clinics, Coastal Healthcare, are looking for a Community Health Licensed Practical Nurse (CHLPN) to join their team. The person in this role will provide direct patient care supporting Provider Care Teams in the Primary Care clinics. Assist the Provider in performance of procedures including wound care, immunizations, and medication administration, point of care testing, telephone prescription orders/refills, medication prior authorizations, maintaining patient Electronic Health Record (EHR) and communication of test results. Provide safe and quality nursing care under the direction of the Provider or Community Health RN within LPN scope of practice. This role is also responsible for promoting health through education and care coordination to optimize patient health outcomes.

$28.29 - $30.01 per hour*

Community Health Nurses will follow the six standards of practice: Assessment – Collect comprehensive data related to the health of the population served. Document assessment data in terms that are understandable to all involved in the care process. Diagnosis and Priorities – Analysis of the assessment data to derive a nursing diagnosis and priorities. Document diagnoses or concerns in a manner that facilitates the population’s involvement in developing a plan of care. Outcome identification – Based on the diagnosis the CHLPN will identify outcomes that are culturally relevant, health literate, based in evidence and attainable through local resources and policies. Document expected outcomes and measurable objectives.  Planning – Develop a plan that is client-centered and reflects best practices by identifying strategies, action plans and alternatives to meeting expected outcomes.  Document the plan using health literate and culturally competent language that can be understood by care team and population being served. Implementation – Oversee the implementation of the plan through treatment, case management, referrals to community partners, collaboration with community resources, health promotion and education. Document activities and status of care coordination. Evaluation – Apply on-going assessment data to revise plans, interventions, and activities as appropriate for ultimate health outcomes for the population. Document changes to enhance effectiveness of interventions.

For Primary Care and Immunization Activities the CHLPN will perform additional job responsibilities: Apply theories and principles of primary care disease prevention and chronic disease management.  Provide direct care services including wound care, immunizations, and medication administration, and point of care testing. Facilitate telephone prescription orders/refills and medication prior authorizations.  Maintain patient EHR record and communication of test results.

Other functions: Educate patients and/or family members on chronic disease management such as Hypertension, Diabetes, COPD or Asthma and health maintenance items. Consistently set appropriate priorities in providing quality patient care that is organized and time efficient. Utilize staff feedback in patient care. Maintain and ensure confidentiality of patient's conditions, records, and other data on a consistent basis.  Assist in clinic operations supporting the Care Teams under the direction of the Provider and the Program Manager. Meet training requirements and practices according to OHA contract.  Follow Clinical Laboratory Improvement Amendments (CLIA) lab policy and protocols. Follow medication dispensing and immunizations policies. Establish professional rapport with clients, community groups and health care partners. Other duties as assigned.

EDUCATION

Completion of a Licensed Practical Nursing certificate program.

EXPERIENCE

Two-Three years LPN experience. 

Substitution Note: Practicums in Nursing programs may substitute years of experience.

Driver’s License

Oregon Driver's License required within 30 days of hire.

Other Licenses

Must possess a valid license to practice in the State of Oregon as Licensed Practical Nurse as defined by statutes or administrative rules, which include but are not limited to ORS 678.010 to 678.360 at the time of appointment. The employee is required to maintain current and active licensures and certifications, as applicable, and to provide the appropriate verifications at the time of renewals.
