Nurse Practitioner - Geriatric Care
SCHEDULE:

COMPENSATION:
- $125,000 to $170,000 Annual Income
- $105,000 to $125,000 Base Salary
- $20,000 to $45,000 Annual Bonus Income, paid quarterly

They currently boast a team of more than 50 physicians and nurse practitioners across Ohio and northern Kentucky, who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include primary care, psychiatric care, post-acute care, and transitional care.
POSITION DESCRIPTION:
The nurse practitioner serves as the primary care provider for a dedicated panel of geriatric patients located in long-term care facilities in Norwalk. The new provider will primarily serve one (1) facility, but may need to see patients in other nearby facilities on a limited basis.
Providers perform an average of 40 to 60 visits per week and dedicated team members put together daily schedules to ensure efficiency and appropriate time for every patient.
Specific duties of the role include, but are not limited to:
- conduct health assessments
- develop personalized care plans
- evaluate patient conditions and responses to treatment
- adjust treatment plans as necessary
- perform minor procedures and wound care (as needed and as settings permit)
- educate patients and families on health management, disease prevention, etc.
- provide guidance on lifestyle modifications and self-care practices
- work with physicians, nurses, and other professionals to coordinate care
The practice uses a mobile-based EHR with AI functionality to reduce administrative time.
On-call is phone triage only with low volume. Providers are typically on call one (1) night per month.
The practice offers a thorough training program and high level of support, welcoming new graduates interested in serving this vulnerable patient population.
REQUIREMENTS:
- Ohio NP license
- DEA
- new grads with backgrounds in home health, LTC, ER, etc. are encouraged to apply
- no experience required
