JOB DESCRIPTION: HIM Clerk II
Department: Health Information Management
Reports To: Health Information Management Manager
JOB SUMMARY
Summary: To collect, maintain, and make available to authorized users timely, accurate, and secure and complete patient health information in a hybrid environment. Will act as an advocate for hospital programs and adheres’ to the hospital Mission, Vision, Values and Motto of “The Patient is First in All We Do”.
Primary Job Duties and Responsibilities:
- Chart Analyst – Reviews each encounter for completeness and for incomplete or missing documentation and signatures, identifies the responsible practitioner/physician and links the deficiencies to the appropriate physician/practitioner.
- Release of Information Technician – Ensures minimum necessary protected health information is used only for purposes identified or as required by law and not further disclosed without patient authorization.
- Deficiency/Delinquency Reporting – Runs and distributes documentation deficiency and delinquency lists to physicians and leadership.
- TruBridge- Responsible for reviewing the daily report and faxing requests for additional documentation and diagnosis.
Secondary Job Duties and Responsibilities:
- Receptionist – Responsible for answering the phones and assisting customers
- Chart Scanning – Scans all batches. Reviews each encounter for completeness, quality and patient identification.
- Indexing – Assures that the patient's medical records are indexed to the highest level of quality.
- Chart completion follow up – Assists with reviewing each encounter for completeness and for incomplete or missing documentation and signatures, identifying the responsible practitioner/physician
- Cross trains in all areas as directed by supervisor and provides coverage as required
- Performs other duties as assigned.

7. Gives appropriate information to external and internal customers with or without request.
8. Seeks out information necessary to perform job functions. Works collaboratively within the team.

Qualifications:
· High school diploma or equivalent.
· One year of medical office experience or related education, preferably in a HIM environment.
· Proper use of telephone etiquette to project a positive image by being clearly spoken, grammatically correct, cordial in tone and conveying a helpful and polite attitude.
· Ability to organize work so that productivity and effectiveness are maximized.
· Must be able to prioritize multiple functions effectively, work independently, self-motivated and self-directed
· Demonstrate excellent organizational skills, written and verbal communication skills, interpersonal skills and strict confidentiality in providing excellent customer service.
· Must be able to adapt to change of duties quickly and smoothly.
· Must uphold high ethical standards.

Requirements
QUALIFICATIONS
Required Job Qualifications include:
- Must be a High School Graduate.
- Experience with office work including typing, computers and office equipment.
- Medical billing experience preferred, but general office experience, with a good understanding of office functions is required.
- Knowledge of billing and collection preferred.
PHYSICAL DEMANDS/REQUIREMENTS
While performing the duties of this position, the employee is required to carry, climb, hear/listen, lift, reach and manipulate objects, tools, or equipment, push/pull, see, speak, squat/kneel, stand, and walk. Position requires moving materials weighing 10 – 20 pounds, and 25 – 50 pounds on an occasional basis. Manual dexterity and coordination are required over 80% of the work period. Work involves the potential for exposure to infection. The employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.
