Job Description:

Physical Therapist Assistant

Make a meaningful difference by helping patients regain strength, mobility, and independence in the place they feel most comfortable—their own homes.

Elara Caring is seeking a compassionate and motivated Home Health Physical Therapist Assistant (PTA) to provide personalized, one-on-one therapy under the direction of a licensed Physical Therapist. You’ll enjoy the independence of working in the field while receiving support from an experienced, collaborative clinical team.

Full-Time | Monday–Friday | Newton-Waltham Territory

What You’ll Do

  • Provide skilled physical therapy services according to the treatment plan established by the supervising Physical Therapist.

  • Help patients improve mobility, strength, balance, and overall function.

  • Review the Plan of Care before each visit and follow physician orders and established clinical guidelines.

  • Monitor patients’ progress and promptly communicate changes in condition to the Physical Therapist.

  • Teach patients and caregivers how to safely complete prescribed home exercise programs.

  • Provide instruction on the safe use of walkers, canes, wheelchairs, prosthetics, orthotics, and other assistive devices.

  • Accurately document visits, patient progress, and response to treatment.

  • Follow all state practice requirements and organizational policies.

  • Collaborate with the Physical Therapist and interdisciplinary care team to support positive patient outcomes.

What You’ll Need

  • Graduate of an accredited Physical Therapist Assistant program.

  • Current, unrestricted PTA license in the state of practice.

  • At least six months of PTA experience preferred.

  • Home health experience preferred.

  • Reliable transportation, a valid driver’s license, and current automobile insurance.

  • Ability and willingness to travel throughout the assigned territory.

  • Sit, stand, bend, lift and move up to 50 pounds, including positioning or transferring patients and moving equipment
