Job Title: Home Health Physical Therapist (PT)
Job Summary:
We are seeking a compassionate, autonomous Home Health Physical Therapist (PT) to deliver high-quality physical therapy interventions directly in patients' homes. In this full-time role, you will perform comprehensive clinical assessments, develop individualized plans of care, and guide patients through rehabilitative treatments to restore mobility, alleviate pain, and prevent hospital readmissions.
This position is ideal for a self-directed clinician who excels at interdisciplinary care coordination, patient and family education, and facilitating safe, independent living.
Key Responsibilities:
Patient Care & Care Coordination:
- Assessments & Care Planning: Perform initial and comprehensive assessments in accordance with clinical standards; develop, update, and coordinate individualized plans of care under physician guidance.
- Therapeutic Interventions: Provide skilled physical therapy treatments, modalities, and procedures to relieve pain, restore functional performance, and improve patient outcomes.
- Patient & Family Education: Educate patients and caregivers on disease management, safe equipment usage, medication awareness, and home exercise programs to promote independence and safety.
- Clinical Supervision & Delegation: Direct and supervise Home Health Aides and Physical Therapy Assistants (PTAs), ensuring all care aligns with the established plan of care.
- Discharge & Emergency Planning: Establish achievable discharge goals from intake and proactively assess emergency department/hospitalization risks to prevent adverse health events.
Documentation & Compliance:
- OASIS & Documentation: Complete timely, precise clinical documentation, time/travel entries, and OASIS assessments in accordance with CMS regulations and agency standards.
- Physician Orders & Communication: Obtain, document, and relay physician orders promptly, maintaining continuous communication with the interdisciplinary healthcare team.
Schedule & Pay Overview:
- Pay Range: $35.00 – $52.00 / hour
- Sign-On Bonus: Up to $10,000 available for qualified external candidates
- Schedule: Full-Time (1.0 FTE), Day Shift (Monday – Friday, 8:00 AM – 4:30 PM)
Candidate Requirements & Qualifications:
Education & Licensure:
- Education: Graduate of an accredited Physical Therapy education program approved by CAPTE (or equivalent approved international education) [Required].
- Licensure: Active, unrestricted State License as a Physical Therapist (PT) [Required].
- Examination: Successful completion of the National Physical Therapy Examination (NPTE) [Required].
Experience & Core Competencies:
- Clinical Experience: Minimum of 1 year of physical therapy experience caring for a similar patient population [Required].
- Home Health Proficiency: Demonstrated ability to conduct physical therapy assessments and treatments in a home setting without direct supervision [Preferred].
- Documentation & Tech: Working knowledge of OASIS data collection, basic Microsoft Office tools, and electronic documentation systems [Preferred].
- Transportation: Must maintain a reliable mode of transportation, a valid driver’s license, and the ability to drive safely in various weather conditions [Highly Preferred].
