The LPN offers support for our prescribers, delegation and support for medical assistants, and provides nursing services for clients within the Health Center Program and the community. Duties will include and not limited to: Client education and support, phone call triage, medication administration/management, lab testing and obtaining vital signs. This licensed provider will engage as a member of a collaborative team. With a Trauma Informed approach, this position will be a representative for the organization and how we offer integrated services for recovery, physical and mental health.
More specifically, the LPN will be responsible for:
- Provide trauma informed services to clients in office, community, and virtually. Deliver positive results as evidenced by utilization of outcome measures.
- Provide accurate, concise, and concurrent documentation according to program procedures and policies. Including, but not limited to the electronic medical record.
- Provide consultation and collaboration with community partners and clients treatment teams.
Position Requirements:
- Graduate of accredited school of licensed practical nursing (required)
- Licensed in the State of Ohio as LPN or RN
- 1 years experience working as a licensed LPN
- Experience working within a behavioral health setting a plus
- QMHS within 90 days of hire
