Hospice Aide, Float Per Diem - Summary
The Hospice Aide, Float Per Diem is a member of the interdisciplinary care team who provides personal care and support services to hospice patients in both the Care Dimensions Hospice House (CDHH) and Kaplan Family Hospice House (KFHH). The role reports to the Clinical Manager or Clinical Director and works under the direct supervision of the RN Case Manager or designee. The position requires a minimum commitment of 32 hours per month and one holiday per year.
Key Responsibilities
- Provide personal care services such as bathing, dressing, toileting, feeding, grooming, and ambulation assistance.
- Assist with exercises as directed by clinical staff.
- Perform supportive services including meal preparation, light housekeeping, laundry, and errands when caring for patients in the home setting.
- Observe and report changes in patient condition to the nursing team.
- Complete required documentation and maintain accurate patient records.
- Collaborate effectively with patients, families, and interdisciplinary team members.
- Participate in required training, continuing education, and mandatory in-service programs.
- Maintain compliance with agency policies, regulatory requirements, and confidentiality standards.
Qualifications
- High school diploma or equivalent.
- Completion of a Massachusetts Department of Public Health-approved Home Health Aide program.
- Home health aide experience preferred.
- Ability to work flexible schedules, including days, evenings, nights, weekends, and holidays.
- Valid driver's license and reliable, insured vehicle.

The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following: education, training, relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post.
