Job Summary and Responsibilities
As our Physical Therapist, you will help patients rebuild or maintain their physical wellbeing so they can function at their best level, reduce pain, and improve their quality of life.Every day you will perform initial evaluations, develop and update individualized plans of care, and provide a variety of direct treatments. You will be expected to consistently meet patient care productivity standards, collaborate with the interdisciplinary team and families, and promote a positive customer service environment.To be successful in this role, you must possess strong age-appropriate competency skills, excellent communication abilities, and a collaborative, patient-centered approach to care.
- Interviews patient, family, caregivers, significant others to obtain information about prior level of function, home environment, family relationships, health and psychosocial history.
- Performs client tests, measurements and evaluations as indicated for the patient’s condition.
- Establishes measurable/functional treatment goals.
- Plans and prepares written treatment programs based on evaluation, age-related and diagnosis- related policies. Administers treatments as prescribed in the treatment plan. Evaluates effectiveness of treatment and adjusts the treatment to achieve maximum benefits.
- Monitors all patients under his/her care: supervises Physical Therapist Assistants and/or Technicians who are involved with treatment, and assures appropriate treatment and documentation; submits a modified plan of care to the physician for altered plans of care; reports any adverse reactions to treatment or changes in condition to the physician as indicated.
Job Requirements
Required
- Graduate of an accredited Physical Therapy program
- Oregon Physical Therapist License
- AHA BLS within 30 days
Preferred
- Clinical experience dealing with a broad based rehabilitation facility
