Day Shift: Monday - Friday 7:00 am - 3:00 pm, rotating on-call responsibilities
Annual Salary: $52,278
Sign-on Bonus: $3,000

The Hampton-Newport News Community Services Board (H-NNCSB) seeks a compassionate, dependable Residential Nurse (LPN) to support individuals with intellectual and developmental disabilities in our Intermediate Care Facilities (ICFs) and Waiver Homes.
In this role, you'll provide hands-on nursing support while coordinating medical care, overseeing medication practices, and partnering with individuals, families, healthcare providers, and residential staff. Your expertise will help promote health, safety, independence, and quality of life for the people we serve.
Required qualifications:
- Graduation from an accredited Licensed Practical Nurse Program and hold a valid LPN license with the Virginia Board of Nursing.
- One (1) year of clinical experience including medication administration in a community program, hospital, and nursing home setting. Previous ICF/IDD/REACH and/or Mental health experience preferred.
What You'll Do
- Conduct nursing assessments, develop and implement care plans, monitor health status, and respond to medical or behavioral health concerns.
- Coordinate medical appointments, referrals, transportation, provider communication, and follow-up care.
- Oversee medication management, including safe storage, administration, documentation, and medication disposal.
- Provide clinical guidance, education, and support to residential staff and families while participating in interdisciplinary service planning.
- Maintain accurate clinical records and ensure compliance with health, safety, environmental, and infection-control standards.
