Human Touch Home Health is seeking an experienced Registered Nurse (RN) Clinical Manager to provide hands-on clinical leadership for our Richmond, Virginia office.
We are looking for a nurse leader who understands home health, can effectively manage a clinical team, knows the importance of compliance and quality documentation, and can balance accountability with support.
This is a full-time, in-office leadership position.

Candidates must meet the following minimum requirements:

Active Registered Nurse (RN) license in Virginia OR an active multistate/Compact RN license that permits practice in Virginia

Minimum 3 years of nursing management or clinical leadership experience in Home Health, Home Care, or a closely related healthcare setting

Strong knowledge of skilled home health clinical operations

Ability to work 100% onsite in Richmond, Virginia

Ability to work Monday through Friday, 8:30 AM–5:00 PM
The hours and onsite requirement are non-negotiable.
This position is not remote or hybrid.

What You'll Do
As the Clinical Manager, you will provide daily clinical leadership and oversight while helping ensure patients receive safe, timely, compliant, and high-quality home health services.
Responsibilities include:

Provide daily leadership and supervision of the clinical team
-
Support and manage RNs, LPNs, therapists, and other clinical staff
-
Monitor clinical staff performance, productivity, and patient-care responsibilities
-
Review clinical documentation for accuracy, completeness, quality, and compliance
-
Provide oversight of OASIS documentation and requirements
-
Review Start of Care, Resumption of Care, Recertification, and Discharge documentation
-
Review and monitor patient plans of care
-
Ensure physician orders are obtained, followed, and appropriately documented
-
Monitor changes in patient condition and ensure appropriate clinical follow-up
-
Assist with case conferences and interdisciplinary care coordination
-
Monitor patient census and clinical staffing needs
-
Help ensure patients are assigned to appropriate clinicians and visits are completed timely

Address missed visits, delayed care, and other clinical concerns
-
Provide clinical guidance and support to field staff
-
Coach, mentor, educate, and hold clinical employees accountable

Participate in clinical orientation and ongoing staff education
-
Assist with competency evaluations and performance management
-
Support quality assurance and performance improvement initiatives
-
Help maintain survey readiness and regulatory compliance

Work closely with the Administrator, Intake, Staffing, Recruitment, and other departments

Utilize WellSky for clinical oversight, documentation review, and workflow management

Required Qualifications

Active Virginia RN License OR active Compact/Multistate RN License valid for practice in Virginia

Minimum 3 years of nursing management or clinical leadership experience in Home Health, Home Care, or a closely related healthcare setting

Strong knowledge of skilled home health operations

Knowledge of OASIS and Medicare home health requirements

Experience supervising clinical staff
-
Strong clinical judgment and critical-thinking abilities
-
Demonstrated leadership and performance-management skills
-
Excellent verbal and written communication
-
Strong documentation and compliance knowledge

Excellent organizational and time-management skills
-
Ability to manage multiple clinical priorities in a fast-paced environment

WellSky experience strongly preferred

Our clinicians need leadership they can reach.
Our patients need clinical concerns addressed quickly.

This Is a 100% Onsite Position

Work Setting: 100% Onsite
These requirements are firm:
The 8:30 AM–5:00 PM schedule is required.
Daily onsite presence in the Richmond office is required.
Remote work, hybrid work, and alternate schedules are not available for this position.
