Provides exceptional patient care and seamless clinical support. Develop and implement community health education. Is confident in triaging telephone calls from patients and medical providers with medical questions or concerns, ensuring timely and appropriate follow-up. Is highly organized and detail-oriented, coordinating patient communications such as lab results, medication refills, dosage changes, and provider instructions with accuracy and professionalism. Thrives in a collaborative team environment and has strong communication skills, empathy, and a commitment to delivering quality care to every patient served. Ensures teams meet their goals in the areas of quality, service, regulatory/accreditation (as applicable), and cost and ensures these goals are compatible with the organization’s mission, vision, values, and strategic initiative.

Requirements

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  1. Creates and provides community education monthly at a variety of locations.
  2. Obtain blood pressure for community members at monthly blood pressure clinics at a variety of locations. 
  3. Respond to patients and providers promptly, document all calls, and maintain accurate, complete records in the patients’ chart.
  4. Enter patient prescriptions and medical orders into patient charts. 
  5. Generate patient and agency related reports, as needed
  6. Participate in agency performance improvement programs and disease management programs.
  7. Assist in the coordination and implementation of staff and community immunization programs
  8. Assist Director of Home Health and Hospice with a variety of office tasks daily
  9. Review patient care problems with staff and promote constructive problem solving.
  10. Ensure staff compliance with state, federal insurance and OSHA regulations.
  11. Assist with staff education and tracking
  12. Assist with ensuring that the QAPI Program is data-driven and implemented company-wide; defines data elements to be captured for ongoing
  13. Attend Clinical Staff team meetings.
  14. Promotes effective communication between programs and different disciplines to provide coordination and continuity of care.
  15. Promote teamwork among teams and other departments. Promotes self-directed work teams. Promotes and encourages professional growth.
  16. Accepts personal responsibility for annual continuing education to keep current in field of practice
  17. Demonstrates commitment to the organizations and network’s mission, vision, values, and strategic initiatives to staff and customers.
  18. Perform other duties as assigned.

EDUCATION AND EXPERIENCE  

  • Associate’s degree required. 
  • Current Licensed LPN with at least 2 years' experience in the field is required.

REQUIRED SKILLS AND ABILITIES

  • Ability to travel required. 
  • Possess a valid drivers’ license required.
  • Possess proof of automobile insurance required. 
  • Ability to pass a Motor vehicle background screening.
  • Strong written and oral communication skills in person and over the phone
  • Strong team skills.
  • Ability to be organized and flexible.
  • Ability to respond well to changing priorities and conditions 
  • Ability to work independently and take initiative.
  • Excellent judgement and problem-solving skills
  • Excellent and ongoing knowledge of Federal and State regulations and ACHC standards.
  • Knowledge of the duties and functions of the home health and hospice agency and its relationship to the healthcare community.
  • Comprehensive knowledge of state and federal guidelines and regulations.

PHYSICAL DEMANDS/WORK ENVIRONMENT

Job requires some physical activities including but not limited to; sitting or standing for extended periods of time, typing, reaching, standing, walking, kneeling, lifting, fingering, grasping, feeling, talking and hearing. 

  • Job requires use of general office equipment including phones, copiers and computer equipment.
  • Job requires light physical work including the ability to exert up to 20 lbs. of force occasionally, 10 lbs. regularly.
  • Job requires visual acuity including the ability to see at or within arm’s reach as well as to operate a motor vehicle.
  • Job is subject to inside environmental conditions; low noise level.
