Patient opportunities may be available throughout:

Lehigh County: Allentown, Whitehall, Emmaus, Macungie, Center Valley, and surrounding communities

Northampton County: Bethlehem, Easton, Nazareth, Hellertown, Bath, Wind Gap, Bangor, Pen Argyl, and surrounding communities

Eastern Berks County: Kutztown, Fleetwood, Topton, Mertztown, Bowers, and nearby communities

You do NOT have to cover the entire territory.

What You'll Do

• Complete skilled nursing visits in patients' homes

• Perform comprehensive nursing assessments

• Complete Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned

• Provide wound care and medication management

• Monitor changes in patient condition

• Provide disease-management education

• Educate patients, families, and caregivers

• Coordinate care with physicians, therapists, and other members of the interdisciplinary team

• Help reduce unnecessary hospitalizations through proactive patient care

• Complete accurate and timely documentation using WellSky

What We're Looking For

• Active Pennsylvania Registered Nurse License Required

• Graduate of an accredited nursing program

• Valid Driver's License and reliable transportation

• Strong assessment and clinical decision-making skills

• Excellent communication and organizational skills

• Ability to work independently in patients' homes

• Home Health and OASIS Experience Preferred, but Not Required

• WellSky Experience is a Plus
