Description
- Perform initial and ongoing comprehensive nursing assessments.
- Develop, implement, evaluate, and update individualized plans of care in collaboration with physicians and other members of the care team.
- Provide skilled nursing services according to physician orders and the established plan of care.
- Monitor patients for changes in condition and communicate concerns promptly to physicians and other appropriate members of the care team.
- Provide patient and caregiver education regarding medications, disease management, treatments, safety, and self-care.
- Perform skilled nursing procedures and treatments within the RN scope of practice.
- Complete OASIS assessments, reassessments, resumptions of care, transfers, discharges, and other required documentation accurately and within required timeframes.
- Coordinate services with Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work, Home Health Aides, physicians, and other healthcare providers.
- Obtain and process physician orders and communicate changes in the patient’s needs or plan of care.
- Help patients and caregivers understand their care plan and work toward their individualized goals.
- Complete accurate and timely clinical documentation for each visit.
- Supervise LPNs and Home Health Aides as appropriate.
- Participate in care conferences, quality improvement activities, and team communication.
- Participate in the on-call rotation as scheduled.
- Follow agency policies, professional nursing standards, and applicable home health requirements.
- Current Registered Nurse license in the State of Utah.
- Graduate of an approved professional nursing program.
- Nursing experience preferred; home health experience is a plus.
- Strong nursing assessment and clinical judgment skills.
- Ability to work independently and make appropriate clinical decisions in the home setting.
- Strong organization, time-management, and prioritization skills.
- Excellent communication and patient-education skills.
- Comfortable coordinating care with physicians, therapists, caregivers, and other healthcare professionals.
- Ability to complete accurate and timely clinical documentation.
- Experience with OASIS is preferred, or a willingness to learn.
- Dependable, flexible, and committed to providing excellent patient care.
- Current driver’s license, reliable transportation, and automobile insurance.
