Colonial Vista Post Acute is a 4-star skilled nursing facility located in Wenatchee, Washington dedicated to providing exceptional care and support to our residents in a comfortable and nurturing environment. **Key Responsibilities:**
- Administer medications and treatments as prescribed by physicians.
- Monitor patient vital signs and report changes to nursing staff.
- Assist with daily living activities such as bathing, dressing, and grooming.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Maintain accurate patient records and documentation in compliance with regulations.
**Required Skills and Qualifications:**
- Current and valid LPN license in the state of practice.
- Strong understanding of nursing principles and practices.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team-oriented environment.
- Compassionate and patient-centered approach to care.
