Part-Time – Ashtabula, OH | Full-Time Combined Opportunity – Ashtabula, OH & Erie, PA
Are you passionate about helping individuals connect with the behavioral health, medical, and community resources they need? We are seeking a Behavioral Health Care Manager to join our collaborative healthcare team in Ashtabula, Ohio. This position offers flexibility based on your interests, qualifications, and desired schedule.
Two employment options are available:
- Part-Time: Work as a Behavioral Health Care Manager supporting patients in the Ashtabula area.
- Full-Time: Combine Behavioral Health Care Management responsibilities in Ashtabula with outpatient counseling services in Ashtabula, OH and Erie, PA.
The full-time opportunity is ideal for a qualified behavioral health professional who enjoys both direct clinical work and the opportunity to coordinate care across behavioral health and medical settings.
What You'll Do as a Part-Time Behavioral Health Care Manager
As part of an interdisciplinary care team, you will help patients to access services, overcome barriers to care, and remain engaged in their treatment. Responsibilities include:
- Build supportive relationships with patients
- Support the behavioral and physical healthcare needs of an assigned patient caseload.
- Coordinate care with primary care providers, behavioral health professionals, psychiatric consultants, and other members of the treatment team.
- Help patients navigate healthcare, behavioral health, and social service systems and connect with appropriate resources.
- Conduct health and behavioral health screenings and facilitate referrals.
- Provide brief, evidence-based behavioral interventions, including motivational interviewing, behavioral activation, problem-solving strategies, and other appropriate approaches.
- Maintain contact with patients to support treatment engagement and re-engage individuals who may have fallen out of care.
- Participate in regularly scheduled caseload consultation with a psychiatric consultant.
- Communicate treatment recommendations to medical providers and other members of the care team.
Full-Time Combined Opportunity
- Candidates who meet the appropriate education and clinical qualifications may have the opportunity to combine the Behavioral Health Care Manager position with outpatient counseling responsibilities in Erie, Pennsylvania to create a full-time schedule.
- In the counseling portion of the role, responsibilities may include:
- Providing individual and/or family counseling services based on clinical need.
- Completing clinical assessments and developing individualized treatment plans.
- Providing evidence-based therapeutic interventions.
- Collaborating with other behavioral health and medical professionals involved in the individual's care.
- Connecting clients with additional behavioral health and community resources as appropriate.
- Maintaining productivity and documentation expectations associated with outpatient clinical services.
- This combined position provides a unique opportunity to develop a diverse clinical role that includes integrated behavioral healthcare, care coordination, brief intervention, and traditional outpatient counseling.
Requirements:
As a Behavioral Health Care Manager
- Three years of experience working in a behavioral health, healthcare, human services, care coordination, or related setting preferred.
- Experience working within a Collaborative Care Model (CoCM) or integrated behavioral health setting is a plus.
- Experience working with individuals who may experience barriers to healthcare or behavioral health services.
- Knowledge of behavioral health and community resources.
As a Behavioral Health Care Manager and Mental Health Professional
- Master’s degree in a Mental Health Field
- Current PA license (LPC, LAPC,LCSW or LSW) licensure or on a licensure track.
For both positions:
- Strong communication, organization, and relationship-building skills.
- Ability to effectively coordinate services across multiple providers and systems.
- Ability to work independently while functioning as part of an interdisciplinary team.
- Experience with electronic health records and clinical documentation systems preferred.
- Experience with motivational interviewing, behavioral activation, problem-solving approaches, care coordination, patient navigation, or other brief behavioral interventions is a plus.
