Heartline Staffing is recruiting a Family Nurse Practitioner for Open Door Community Health Centers in Arcata, California. This is a full-time outpatient, panel-based primary care role focused on caring for patients with complex medical needs in collaboration with an integrated clinical team.

The provider will work alongside dedicated medical assistants, RN support, behavioral health, chiropractic services, and telehealth consultation resources to deliver comprehensive, community-centered care.

COMPENSATION
• $148,627.20–$168,553.60 annual base salary
• $12,400 annual access bonus paid biweekly for meeting the stated 9-patient per clinic target
• Up to $44,000 additional annual production bonus potential for higher patient volumes
• $25,000 sign-on plus retention bonuses for partnership-eligible providers
• Alternative sign-on and retention structure available for non-partnership-track providers
• Up to $10,000 relocation reimbursement
• Additional $10,000 relocation assistance for a qualifying parent or in-law relocation

SCHEDULE / PRACTICE
• Full-Time
• 5x8 schedule
• Outpatient, panel-based primary care
• Six-month mentorship and onboarding program
• Integrated team model with 2 Medical Assistants and 0.7 RN support per provider
• Access to behavioral health, chiropractic, and specialty telehealth consultation services

POSITION OVERVIEW
• Provide comprehensive outpatient primary care to patients with complex medical needs.
• Manage a panel of primary care patients within an integrated-care model.
• Collaborate with Medical Assistants, RN support, behavioral health providers, chiropractic services, and telehealth specialty resources.
• Deliver compassionate, team-based care within a community health setting.
• Participate in the organization's structured six-month mentorship and onboarding program.

Requirements

Requirements
• Active California Nurse Practitioner license or eligibility to obtain California NP licensure REQUIRED
• Ability to provide comprehensive outpatient primary care within an integrated, team-based environment REQUIRED

Preferred
• 3+ years of outpatient Nurse Practitioner experience
• Interest in rural health and serving underserved populations
• Spanish language skills; differential pay may be available
