Sentara CarePlex Hospital and Sentara Louise Obici Hospital are seeking a dedicated Inpatient Case Management Supervisor, RN to join our collaborative team. Position Summary
As Case Management Supervisor, you will work across both hospitals, partnering with the Manager and Senior teams to ensure high-quality patient care, effective discharge planning, and efficient case management practices. Your expertise will be vital in supporting staff development and maintaining compliance, while leading a cohesive team focused on achieving exceptional outcomes.
Key Responsibilities:
Supervise the day-to-day workflow and performance of the case management team at both hospital locations
Ensure effective discharge planning and seamless coordination of inpatient care
Support length-of-stay management, patient flow processes, and related performance improvement initiatives
Provide coaching, mentoring, and constructive feedback to staff to foster professional growth
Address employee relations matters in partnership with leadership and HR
Maintain compliance with departmental standards, hospital policies, and regulatory requirements
Requirements
Bachelor’s degree in Nursing (BSN)
Current Registered Nurse (RN) license—either Virginia or Compact/Multi-State
Basic Life Support (BLS) certification from the American Heart Association (required within 90 days of hire)
Specialty certification required within one year of eligibility (acceptable certifications include ACM, CCM, CCCTM, CMAC, or CMGT-BC)
At least 3 years of case management experience (required)
Previous supervisory experience (preferred)
