Description
Physical Therapist Assistant (PTA) – Assisted Living Facility | Port Jervis, NY | Full-Time or Part-Time
- Provide one-on-one physical therapy treatments at a single assisted living facility.
- Carry out individualized plans of care established by the supervising Physical Therapist.
- Help patients improve strength, balance, mobility, endurance, safety, and overall functional independence.
- Provide therapeutic exercise, gait training, transfer training, balance activities, and other interventions outlined in the plan of care.
- Monitor patient progress and communicate changes in condition to the supervising Physical Therapist.
- Educate patients and caregivers on exercises, mobility strategies, and safety.
- Complete accurate and timely clinical documentation.
- Communicate with patients, families, facility staff, supervising PTs, and interdisciplinary care team members.
- Coordinate with Stern's administrative team regarding scheduling and patient care.
- $35–$50 per visit.

- Geriatric, assisted living, skilled nursing, outpatient, or home health experience preferred but not required.
