Position Title: Parenting Care Coordinator II

Employment Classification: Full Time / Hourly / Non-Exempt

Shift: Monday - Friday 9:00am - 5:30pm 

Reports to: Housing Supervisor

Department: Housing


Program Summary: 

Casa De Rosemary and OrangeTree are a housing continuum for transitional age young adults (TAY) single parents and their children, providing up to three years of housing support. 


Casa de Rosemary is a bridge housing program that houses 5 mothers ages 18-24, along with their young children aged 0-5, in a shared home environment, with a lower program fee to mimic rent.


OrangeTree apartments are a next step, allowing for TAY mothers and their children, along with TAY dads and their children to pay a pro-rated program fee, to mimic rent, and utilities for their own 1-bedroom apartment. In addition, OrangeTree also serves TAY parents who have shared custody or are in the process of reunification in a shared 2-bedroom unit.


Position Summary: 

The Care Coordinator is responsible for providing direct, person-led support and coordination of care for residents who are also parents. The Care Coordinator facilitates pre case management for potential residents to help transition them into the living space, intake, and post case management to help young adults transition to long term stable housing. The bridge housing programs require weekend and holiday coverage, to help support parenting young adults with higher needs.

Requirements

Major Areas of Responsibility: 

  • Provide direct support and care to residents using a trauma-informed, positive young adults development approach as a core philosophy.
  • Coordinate weekly support sessions based on young adults schedule and desire to participate, review progress on a regular basis to support the successful attainment of skills and goals.
  • Implement a positive young adults development framework to support residents to maintain the home as a safe, young adults-led, home environment, including resident facilitation of interviews with potential new residents and feedback to inform the programming and guidelines of the program.
  • Support residents to develop an awareness and understanding of individual and child safety using motivational interviewing techniques
  • Plan, coordinate and motivate young adults to engage in support services based on young adults needs, to support the successful obtainment of goals.
  • Implement Stages of Change frameworks to support residents to maintain a safe, home environment, including feedback from residents to inform the programming and guidelines of the program.
  • Support residents to develop an awareness and understanding of individual safety and continually safety plan using a harm reduction approach.
  • Provide crisis intervention and conflict resolution as needed, using therapeutic interventions that encourage and empower residents to develop and use their own communication skills to discuss the issue causing the conflict. 
  • Work collaboratively with young adults identified supports, communicating about shared clients to meet the needs of young adults receiving services from all programs. 
  • Review program applications and participate in the intake process, participation in interviews with potential new residents and assist with decisions regarding a young adults readiness for the program. Coordinate the young adults move in process, logistics of their move-in and ensure the emotional needs of young adults and child are met. 
  • Support the young adults’ move out process and connect young adults to aftercare services to help young adults sustain the skills needed to maintain their housing.
  • Provide collaboration and advocacy with other service providers on behalf of residents and attend case conferences and court as needed. 
  • Schedule and co-facilitate/facilitate team meetings including swing and overnight staff, outside providers, volunteers, interns or individuals who are part of the resident’s support system.
  • Maintain confidentiality related to program participation. 
  • Additional duties as assigned by program leadership.

Education and Experience Requirements:

  • A High School diploma, Bachelor’s degree preferred in a major relevant to the work or equivalent is required
  • 2+ years in the field of Human Services, Psychology, Social Work or a related field, specifically serving individuals who are parents.
  • Fully proficient professional who independently manages a full workload and consistently demonstrates sound judgment.

Knowledge and Skills:

  • Knowledge of the needs and challenges faced by young adults who are parents, including but not limited to stigma, lack of trust and protective behaviors. 
  • Knowledge, understanding, and experience of implementing interventions from a person centered, trauma informed and harm reduction approach
  • Develops strong therapeutic and professional relationships, effectively engages participants experiencing resistance or ambivalence, and adapts engagement strategies to individual needs. 
  • Independently evaluates situations, identifies solutions, anticipates barriers, and resolves most participant issues without supervisor involvement. 
  • Independently manages a full caseload while coordinating services across multiple agencies, systems and sites.
  • Produces consistently high-quality documentation with minimal corrections. 
  • Demonstrates understanding of compliance requirements. 
  • Effectively balances competing priorities and consistently meets timelines. 
  • Builds productive working relationships across departments and community partners.
  • Identifies opportunities for improvement and acts without direction. 
  • Acts as an informal resource and role model for peers. Strong communication, documentation, and interpersonal skills.
  • Commitment to support young adults in their recovery and personal development.
  • Ability to remain calm and make sound and timely decisions under pressure, particularly when managing young adults' crises.
  • Maintain confidentiality and demonstrate ethical decision-making in all aspects of the role.
  • Ability to think critically and strategically, identifying opportunities and challenges and developing solutions to support young adults' success.
  • Must be computer-literate and be able to understand and effectively use electronic health record/database systems

Working Environment/Physical Requirements: 

  • Position will require use of personal vehicle. 
  • Primary work environment is multiple housing sites, including homes and apartments, where participants live.
  • Physical requirements include standing, sitting, typing, bending and lifting up to approximately 25 lbs. (i.e. tables, chairs, food, water, etc.). 
  • Modification to work schedule may be made based on needs of the Foundation.
  • Possess a valid California driver’s license and proof of automobile insurance. Position requires DMV clearance.
