Overview:

We are seeking a dynamic and compassionate Director of Dementia Care to lead our specialized memory care programs. This pivotal role involves overseeing the development, implementation, and continuous improvement of dementia-specific care strategies within our long-term care facilities. The ideal candidate will possess extensive experience in nursing home environments and managing care teams dedicated to residents with memory impairments. Your leadership will ensure that our residents receive respectful, personalized care that upholds their dignity and rights while complying with all regulatory standards.

Responsibilities:

- Lead the design and execution of comprehensive dementia care programs tailored to residents’ unique needs, emphasizing person-centered approaches.
- Supervise and mentor nursing staff, care coordinators, and support teams to ensure high-quality resident care and adherence to best practices in memory care.
- Oversee the development and review of individualized care plans that incorporate residents’ medical history, physiology, anatomy knowledge, and personal preferences.
- Ensure compliance with all long-term care regulations, HIPAA privacy standards, Medicare guidelines, and other regulatory requirements related to dementia and memory care.
- Manage documentation review processes including medical records, care documentation, MDS (Minimum Data Set), and clinical notes within EMR/EHR systems for accuracy and completeness.
- Facilitate staff training on topics such as resident’s rights, medical terminology, documentation procedures, and specialized dementia care techniques.
- Collaborate with interdisciplinary teams to optimize patient outcomes through effective shift management and resource allocation.

Qualifications:

- Must have earned a degree from and educational institution in a healthcare, mental health, or social service profession or be a licensed health facility administrator.
- Must have a minimum of one year work experience with Dementia or Alzheimer's residents, or both, within the past five years.
- Proven leadership experience managing long-term care facility teams with a focus on dementia or memory care programs.
- Extensive assisted living experience coupled with nursing home or hospice care background.
- Strong understanding of long-term care regulations, Medicare policies, and regulatory compliance standards.
- Proficiency in EMR/EHR systems for clinical documentation review and management of medical records.
- In-depth knowledge of medical terminology, physiology, anatomy relevant to aging populations with cognitive impairments.
- Demonstrated ability to develop effective care plans tailored to residents’ individual needs while ensuring compliance with HIPAA standards.
- Supervising experience in a healthcare setting with a focus on nurse management or nursing administration roles.
