JOB DESCRIPTION:

Must possess and maintain a current, unrestricted professional clinical license in the applicable state, including one of the following:

  • Licensed Independent Social Worker (LISW)
  • Licensed Clinical Social Worker (LCSW)
  • Licensed Mental Health Counselor (LMHC)
  • Licensed Professional Counselor (LPC)
  • Licensed Professional Clinical Counselor (LPCC)

SUMMARY OF POSITION

Responsible for the coordination, development, implementation, marketing, organization, and management of assigned program areas, including supervision of staff, interns, and practicum students as assigned. Assists in ensuring compliance with organizational standards, grant requirements, regulations, and applicable laws while providing and promoting outstanding internal and external customer service.


The Grant Coordinator for the Promoting Integration of Primary and Behavioral Health Care (PIPBHC) program oversees the implementation and daily operations of an integrated care grant that coordinates behavioral health, primary care, and community support services. This position is responsible for ensuring grant objectives, deliverables, performance measures, and compliance requirements are achieved while promoting high-quality, person-centered care.


Key responsibilities include supervising program staff, monitoring program performance and outcomes, managing grant-related budgets and reporting requirements, coordinating cross-functional partnerships, and ensuring adherence to federal, state, grant, and organizational regulations. The position collaborates with organizational leadership, healthcare providers, community agencies, and other stakeholders to improve access to care, enhance service integration, and support positive client outcomes.


The Grant Coordinator analyzes program data, identifies opportunities for operational and service improvements, supports quality assurance and performance improvement initiatives, and assists with strategic planning and sustainability efforts related to integrated care services. Strong leadership, communication, grant management, healthcare coordination, and program oversight skills are essential for success in this role.


This position serves as the replacement for the Clinical Manager role and maintains responsibility for program leadership, staff supervision, operational oversight, compliance, and service excellence within assigned areas.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Provides clinical supervision and consultation to clinicians and interns/practicum students, as assigned to assure quality of services. Assists with training new staff/Interns/practicum students.
  • Make necessary decisions in the absence of Clinical Manager to assure program continuity.  Act in consultation with other Managers and Director as needed.
  • Provides outpatient counseling services utilizing evidence-based methodologies to provide excellent clinical services.  Prepares for and conducts individual, group, and family therapy for assigned caseload.
  • Assists clients in developing an individualized treatment plan, must be person centered, family centered, and service centered.  Monitors client progress in treatment and participates in treatment plan reviews as assigned.  Updates treatment plans as appropriate. Submits treatment plans at all stages for medical review, if applicable. 
  • Conducts intake for clients entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned. Conducts assessments and write assessment summaries, as assigned.
  • Assists clients with making linkages with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services.  Maintains appropriate contact with referral sources including providing information on client progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate. 
  • Assists client in making discharge plans and provide follow up services as assigned.
  • Attends staff meetings, clinical staffings, and other internal meetings as assigned.  Participates on committees as assigned, including quality assurance functions.  Maintains appropriate communication with other employees and work as an effective member of treatment team.
  • Monitors urinalysis, breathalyzer, and other testing as assigned.

QUALIFICATIONS

Education Level              

Master’s Degree in Human Services


Years of Experience       

Demonstrated skill in performing clinical services


Certification/Licensure 

Clinical License required. SAS staff must have certification or licensure within the required time frame mandated by DASA Staff Qualifications (or within 2 years of hire, whichever comes first).
