Acquisition et mise en application des connaissances et habiletés reliées à l’optimisation de la thérapie médicamenteuse des patients ayants des problèmes de santé aigus ou chroniques légers à modérés. Les cours de gestion thérapeutique sont intégrés et organisés en fonction des systèmes physiologiques majeurs. Ils incluent des notions de pathophysiologie, de galénique appliquée, de chimie médicinale, de pharmacocinétique clinique, de pharmacologie clinique, et de pharmacothérapie.

Requirements:

Minimum d'une formation avancée en gestion clinique du diabète (Spécialité, certificat, formation additionnelle ou équivalent en diabète). Une formation en pharmacie requise (Pharm. D, B.Pharm ou équivalent reconnu). Être précepteur en pharmacie ou tuteur en laboratoire clinique en pharmacie un atout Être un pharmacien en exercice actif et inscrit à un ordre des pharmaciens d'une province canadienne (Ontario, Québec ou autre). Vaste expérience clinique incluant la gestion et le suivi de patients diabétiques en pharmacie communautaire ou hospitalière.
