Sequoia Home Health is growing and looking for experienced, compassionate Home Health Patient Care Coordinator to join our team!
Job Summary
Join our team as a Patient Care Coordinator and help deliver exceptional patient care. This role oversees care coordination, referral and intake processes, clinical documentation review, and communication between patients, providers, and interdisciplinary teams to ensure quality outcomes and regulatory compliance.
Key Responsibilities
Coordinate patient admissions, intake, and referral processes.
Ensure compliance with state, federal, and Joint Commission regulations.
Review clinical documentation to determine home health eligibility.
Collaborate with physicians, referral sources, patients, families, and caregivers.
Support care plan development and coordination of services.
Track hospitalizations, holds, and resumptions of care.
Participate in QAPI initiatives and case conferences.
Assist with scheduling, intake, and urgent clinical support as needed.
Support staff onboarding, training, and continuing education programs.
Build and maintain strong relationships with referral partners.
Qualifications
Minimum 3 years of healthcare experience; home health, community health, or medical-surgical experience preferred.
Strong knowledge of home health regulations, patient care coordination, and clinical operations.
Excellent communication, organization, and problem-solving skills.
Ability to work collaboratively in a multidisciplinary healthcare team.
Valid driver's license, reliable transportation, and current auto insurance.
