As a Clinical Pharmacist, a caregiver in this role will serve as the primary pharmacist supporting the inpatient Hematology/Oncology service within a 256-bed hospital caring for a high-acuity patient population, including a robust bone marrow and stem cell transplant program. The caregiver will participate in multidisciplinary rounds, collaborate with providers on patient care plans and review, verify and optimize chemotherapy and supportive care medication orders in Epic. Responsibilities will include clinical medication monitoring, therapeutic interventions, medication safety and accuracy, drug information consultation and monitoring patients for medication efficacy, toxicity and adherence to treatment protocols. This role will also support oncology and transplant-related medication management and assist with operational pharmacy staffing as needed.

A caregiver in this position will work 8:00am to 4:30pm.

A caregiver who excels in this role will:  

  • Provide safe, efficient, patient-centered and evidence-based pharmacy and medication services.  
  • Ensure the appropriate selection, dosing, administration and dispensing of medication orders.  
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.  
  • Document services in a complete, accurate and timely manner. 
  • Actively contribute to medication safety initiatives. 
  • Ensure all pharmacy operations are in compliance with national accreditation standards. 
  • Educate and train students, residents, other caregivers, and/or patients.  
  • Provide cost-effective care. 

Minimum qualifications for the ideal future caregiver include:  

  • PharmD or Bachelor’s of Science in Pharmacy from an accredited College of Pharmacy 
  • Current state licensure as a Registered Pharmacist (RPh) or eligible in current state within 90 days of hire 
  • For individuals required to administer immunizations, current Basic Life Support (BLS) certification through American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire are required.  For individuals required to respond to adult or pediatric cardiopulmonary arrest events, current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certification through American Heart Association (AHA) or American Red Cross or ALS through American Red Cross, or certification obtained within one (1) year of hire, is required.

Preferred qualifications for the ideal future caregiver include:  

  • Board Certification 
  • Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience 
  • Post-Graduate Year 2 Residency (PCY2) or relevant experience for Clinical Pharmacists in a specialized pharmacy practice area 

Physical Requirements:

  • Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time. 
  • Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.
  • Position often includes reaching, stretching, stooping, climbing steps and sitting.
  • Must have normal or corrected vision.
  • Must have ability to clearly communicate verbally by phone or in person

Personal Protective Equipment:
