Location: Longwood, FL 

Employment Status: Full-Time / Non-Exempt

Pay: $19.23 per hour, ($40,000.00 annually) with higher compensation based on experience and education. 

About the Position

IMPOWER is seeking a Supportive Housing Case Manager to provide individualized case management and housing support to young adults experiencing homelessness. This position manages a caseload of approximately 15–22 young adults participating in Rapid Rehousing (RRH) and Permanent Supportive Housing (PSH) programs and works collaboratively with participants, landlords, service providers, and community partners to promote housing stability and independence.

Key Responsibilities

• Support young adults in identifying and working toward housing, employment, education, and other individualized goals.
• Provide services using Housing First, Trauma-Informed Care, Motivational Interviewing, and strengths-based approaches.
• Complete required assessments and develop individualized housing stability plans.
• Assist with housing searches, placements, move-ins, utilities, and coordination with landlords and other providers.
• Provide flexible, hands-on support to help participants obtain and maintain stable housing.
• Conduct twice-monthly home visits and maintain regular communication with participants to monitor progress, address barriers, and coordinate services.
• Connect participants with community resources, including employment, education, behavioral health, substance use treatment, benefits, and other supports.
• Maintain accurate and timely case notes, assessments, and other required documentation in HMIS and applicable program systems.
• Collaborate with landlords, community providers, and other partners to support housing stability and continuity of services.

Qualifications

• Associate degree in social services or a related field; bachelor’s degree preferred.
• At least two years of case management experience, preferably working with youth or young adults.
• Knowledge of homelessness, housing programs, community resources, and supportive services preferred.
• Ability to work independently in the community and build effective relationships with participants and community partners.

Pre-Employment Requirements

Employment is contingent upon successful completion of all applicable pre-employment requirements, which may include Level II background screening, local law enforcement checks, drug screening, verification of education and credentials, and E-Verify and drug screening. 

A valid driver’s license, proof of auto insurance, and satisfactory Motor Vehicle Report (MVR) are required as applicable to the position.
