Description
Schedule:
Approximately one (1) day per week, with services scheduled between 8:00 AM and 3:00 PM, Monday through Friday, excluding holidays. The exact weekly schedule will be coordinated with the Special Education Coordinator. The Physical Therapist must also be available as needed by email, phone, virtual meetings, and while onsite.
Summary:
The Physical Therapist will provide onsite school-based physical therapy services at Turtle Mountain Middle School in Belcourt, North Dakota. The therapist will support students requiring special education-related services by conducting evaluations, providing direct therapy, developing therapy plans, documenting progress, and collaborating with school staff and parents.
Requirements:
Current, valid, and unrestricted Physical Therapist license in North Dakota.
Must be a United States citizen.
Must be able to provide services onsite.
Knowledge of special education requirements, laws, regulations, and North Dakota reporting timelines.
Graduate of a CAPTE-accredited Physical Therapy program.
Must meet one of the following education/experience requirements:
Bachelor's Degree in Physical Therapy with 5 years of progressively independent school-based PT experience; or
Master's Degree in Physical Therapy with 2 years of progressively independent school-based PT experience; or
Doctorate Degree in Physical Therapy with school-based experience.
Proficient in written and spoken English.
Ability to successfully complete required background and security investigations.
Must maintain required professional licensure throughout the contract.
Must meet applicable health requirements, including required immunization documentation and annual negative TB testing.
Duties:
Provide direct onsite physical therapy services to students.
Conduct screenings, evaluations, re-evaluations, and assessments.
Develop and implement individualized therapy plans with measurable goals.
Maintain a direct service log for each student and each visit.
Collaborate with the Special Education Coordinator to establish the weekly service schedule.
Consult with teachers, paraprofessionals, school staff, and parents regarding student needs and progress.
Complete required evaluations and written reports within established timelines.
Attend and participate in multidisciplinary team meetings.
Provide recommendations regarding student equipment needs.
Prepare quarterly written reports documenting student progress.
Provide training, guidance, and ongoing support to school staff and parents.
Maintain accurate documentation while protecting confidential student information.
Provide services professionally, respectfully, and in a manner that protects student dignity and rights.58316
