Iowa City Hospice is seeking a compassionate and dedicated part time Hospice Social Worker (MSW) to join our interdisciplinary care team. In this meaningful role, you will provide psychosocial support, counseling, advocacy, and resource coordination for patients and families facing life-limiting illnesses. Through compassionate guidance and skilled support, you will help individuals navigate the emotional, social, and practical challenges that accompany the end-of-life journey.

Essential Responsibilities

  • Conduct comprehensive psychosocial assessments of patients and families.
  • Develop individualized care plans based on identified psychosocial needs.
  • Provide emotional support, counseling, and crisis intervention to patients, families, and caregivers.
  • Facilitate discussions regarding coping strategies, grief, loss, end-of-life concerns, and advance care planning.
  • Connect patients and families with community resources, financial assistance programs, support groups, and other services as needed.
  • Collaborate with physicians, nurses, chaplains, hospice aides, and other interdisciplinary team members to ensure holistic patient care.
  • Participate in care conferences and contribute to care planning and goal setting.
  • Assist families with discharge planning, placement options, and transition of care when appropriate.
  • Maintain accurate, timely, and compliant documentation in the electronic medical record.
  • Provide bereavement support and follow-up as assigned.

Requirements


  • Bachelor's Degree in Social Work from a CSWE-accredited program required; Master's Degree preferred.
  • Current Iowa LMSW, LISW, or LBSW licensure preferred.
  • Minimum one year of medical social work, counseling, or related healthcare experience preferred.
  • Full-time availability, Monday through Friday, 8:00 AM to 5:00 PM.
  • Valid driver's license and current automobile insurance required.
  • Knowledge of medical terminology, healthcare systems, community resources, and social service programs.
  • Strong communication, interpersonal, organizational, and time-management skills.
  • Ability to work independently while collaborating effectively as part of an interdisciplinary team.
  • Experience conducting psychosocial assessments, care planning, counseling, and patient advocacy.
  • Compassionate, empathetic, and patient-centered approach to care.
  • Ability to connect patients and families with appropriate community resources and support services.
  • Demonstrated professionalism, sound judgment, and commitment to maintaining accurate and confidential documentation.
