Description
Cayuga Counseling Services is seeking a Non- Medicaid Care Manager in Cayuga County to provide person centered care coordination and support to individuals and families who need assistance accessing and coordinating community, health, behavioral health, educational and social services.
The Care Manager is responsible for the following:
- Complete assessments to identify individual and family strengths, needs, barriers and available supports.
- Develop individualized service goals in collaboration with the individual and family.
- Coordinates and monitor services across healthcare, behavioral health, education, social services and community programs.
- Make referrals and follow up to ensure individuals successfully connect with the services needed.
- Maintain regular contact with assigned individuals and families and conduct outreach when engagement is limited.
- Assist individuals and families with navigating complex service systems and accessing community resources.
- Advocate for individuals and families and promote self-advocacy and independence.
- Collaborate with schools, healthcare providers, behavioral health providers, DSS, community agencies, and other identified supports.
- Maintain current knowledge of community resources and develop effective referral relationships.
- Identify changes in needs, barriers, or safety concerns and coordinate appropriate support or referrals.
- Maintain timely and accurate documentation of contacts, referrals, service plans, and care coordination.
- Participate in case consultation. supervision, staff meetings, training and quality improvement activities.
- Maintain confidentiality, professional boundaries, and compliance with agency policies.
- Performs other duties as needed.EEO
Knowledge, Skills, and Abilities
- A Bachelors of Arts or Science Degree in psychology, sociology, human services or related field.
- Experience in case management, care coordination, social services, behavioral health, or community-based services preferred.
- Strong communication, organization, problem solving and advocacy skills.
- Knowledge of local community resources and service systems preferred.
- Travel is a requirement. Must have valid NYS driver's License
The Agency offers:
- 3 weeks of paid time off in your first year, 4 weeks in your second year.
