  • Conduct assessments of patient strength, balance, range of motion, gait, transfers, endurance, and functional limitations.
  • Develop and implement individualized physical therapy treatment plans based on patient diagnoses, functional goals, and physician-directed care plans.
  • Deliver therapeutic interventions, including strengthening exercises, mobility training, balance activities, gait training, and functional rehabilitation.
  • Assess patients' home environments to identify mobility challenges, safety concerns, and opportunities to reduce fall risks.
  • Provide fall prevention education, transfer training, and mobility assistance to support safe daily activities.
  • Recommend appropriate assistive devices, adaptive equipment, and therapeutic strategies to improve patient safety and functional independence.
  • Monitor patient progress, reassess functional outcomes, and modify treatment plans based on clinical findings and rehabilitation goals.
  • Educate patients, family members, and caregivers regarding home exercise programs, mobility techniques, safety precautions, and rehabilitation expectations.
  • Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other healthcare professionals to coordinate comprehensive home health services.
  • Communicate changes in patient condition, progress toward rehabilitation goals, and recommendations for continued care to the appropriate healthcare team members.
  • Maintain accurate and timely patient evaluations, treatment notes, progress reports, and discharge documentation in accordance with organizational standards.
  • Coordinate patient visits and manage daily treatment schedules to ensure efficient delivery of home health therapy services.
  • Follow established infection prevention practices, patient safety protocols, and applicable Texas and federal healthcare regulations.
  • Promote patient independence, functional recovery, and quality of life through compassionate, evidence-based physical therapy care.
