Position Highlights

The LTC LPN performs an active role in nursing (including passing medications), as well as mentoring staff, assessing residents, communicating with staff, physicians and residents. This position will serve as a resident advocate for quality clinical care. The LTC LPN will ensure completion of care plans and required documentation, uphold regulatory compliance, nurture resident-directed culture, strengthen the ability of the household to grow as a separate effective home and promote individual and team learning in the household. LTC LPNs are teachers and role-models of professional practices to all household staff.   This role will also work cooperatively with social services, activities, dietary services and housekeeping to ensure facilitation of the household model of care. This role will require problem solving, conflict resolution and ensuring that the needs and desires of the residents are met.

REQUIRED EDUCATION and/or EXPERIENCE:

• At least 2 years of long term care experience with strong assessment and communication skills required.
• LPN licensure required
• Certification in Gerontological nursing is preferred, specialized training or experience in geriatric nursing is desirable. 
• Must have demonstrated leadership qualities, supervisory skills, interpersonal relationship skills, and knowledge of federal and state regulations governing clinical care, nursing practice and leadership. 
• Commitment to person centered and resident directed care is essential.

POSITION REQUIREMENTS:

• Annual influenza vaccinations are mandatory for all employees by December 1, unless a medical waiver is signed and approved.
• Completion of the Dependent Adult Abuse course for Iowa Mandatory Reporters within 30 days of employment and every 3 years thereafter.
• Current valid Iowa LPN licensure required; CPR certification also required. 
• Sanitation Certification required.
