- Takes and records the medical history of employees, routine vital signs of all consulting employees which may include BP, PR, RR, Temp., weight, height;
- To observe and provide routine nursing care such as applies compresses, ice bags, hot water bags, and alcohol rubs as needed for all observed company employees. Performs wound dressing preferably under the supervision of the Company Physician.
- Dispenses / administers medicines to patients with proper instructions and keeps a record of the medicines given.
- Reports to the Company Physician employees that need medical attention.
- Contacts thru phone or personally or thru supervisors of employees with incomplete or pending med exams and certification if needed.
- Conducts house / hospital visits as required by the Human Resources Department (HRD) or the Company Physician.
- Assists and accompanies patients to the hospital when needed during referrals. Notifies the relative of sick employees who needs to be referred to the hospital; liaises between hospital personnel and the employee for in-patient requirements and other concerns.
- Assists the Company Physician in the approval of sick leaves and approval of medical leave requests
- Assists the company Physician in verifying the validity of the medical certificates submitted.
- Assists the Company Physician in announcing that an employee is fit to work after sickness.
- Evaluates the condition of sick / injured employees requesting to stay at the clinic to rest; issues a send home order as necessary and informs the employee’s immediate superior and workforce regarding the patient’s condition.
- Makes incident reports on any injury/s and submits this to the Company Physician.
- Makes a report of all referred, observed and sent home cases and clinic visits and submits this to the nurse supervisor or company physician.
- Promotes health through health and wellness activities, counseling and seminars, and health education, vaccination programs. Coordinates with concerned departments in securing work permits.
- Helps in the maintenance of a safe and healthy working environment.
- Assists the Nurse Supervisor in the inventory of medical supplies and update of stocks of medicine in the clinic.
- Reminds the Company Physician of medicines nearing expiration and reports such for proper dispensing and discards medicines and disposes such in the prescribed manner.
- Answers inquiries and concerns regarding HMO benefits.
- Assists in the conduct of the Annual Physical Examination (APE) and coordinates with Facilities for the necessary gate pass.
- Assists the company physician in encouraging employees comply to APE and pre-employment medical examination (for new hires).
- Ensures that all medical consultation, clinic visits and other medical data are logged in appropriate log sheets and worksheets particularly in the health tracker.
- Keeps health records of employees.
- Encodes pertinent medical / health information of employees, including prescriptions, in the Health Tracker to record the employee’s medical history;
- Responsible for computer data entry of patients’ daily visitation log to include all consultations, referrals, meds dispensed and filed sick leaves and other pertinent records like approval or disapproval of leaves, present illness;
- Prepares weekly and monthly clinic report and keeps a record of the reports;
- Ensures the maintenance of cleanliness of the workplace and the clinic; coordinates housekeeping concerns with concerned department; sees to it that proper sanitation procedures are followed to include the disposal of hazardous wastes like blood soaked gauzes, gloves, (sharps and needles) .
- Attends seminars / conventions pertaining to health as part of the continuing medical education as deemed necessary and upon approval of Management.
- Reports to the office 40 to 45 hours a week and do overtime work as the need arise and upon the directive of the company physician. May report to other VXI sites as needed.
- Performs other duties pertaining to maintenance of health and safety and admin tasks that may be assigned from time to time.
