ROLE SUMMARYNetcare is seeking dedicated and motivated Digital Clinical Pharmacists to join our Digital Clinical Pharmacy Unit. As a Digital Clinical Pharmacist, you will be responsible for conducting digital clinical ward rounds and review patient medication profiles across the hospital division. This role provides an excellent opportunity to apply and enhance your clinical pharmacy skills and contribute to improving patient outcomes through pharmaceutical care. The services provided by the Digital Clinical pharmacy unit will support patient centred health and care which is digitally enabled, and data driven using digital platforms to execute high impact clinical pharmacy initiatives aimed at the promotion of rational and appropriate use of medicines and pharmaceutical care. Pharmaceutical care mandates that pharmacists should not only dispense medicines but also assume the responsibility of improving the quality of life of patients and improving therapeutic outcomes.
Pharmaceutical care within a digital lead clinical pharmacy context involves the implementation of the following steps:
(a) Provide advanced clinical pharmacy services to medical specialities.
(b) Monitoring of patient’s response to therapy to ensure optimum therapeutic outcomes.
(c) Act as a leading pharmaceutical partner within a multi-professional health care team.
(d) Performing medicine reviews to detect and resolve medicine-related problems.
(e) Appraise clinical pharmacy information, make informed decisions with the evidence available and be
able to justify/defend the decisions.
(f) Provide education and training related to clinical pharmacy.
(g) Collaborate with pharmacy management and frontline pharmacists (as applicable) for the continuum of clinical pharmacy service at hospital level in the form of meetings or presentation of data in the interest for shared learnings for improvement in patient care
(h) Perform research, teach, and aim to publish in clinical pharmacy.
(i) Develop and maintain professional relationships with key clinical stakeholders for the best interests in best patient care
KEY WORK OUTPUT AND ACCOUNTABILITIES
CONSISTENCY OF CARE
Digital Clinical Pharmacy Care Rounds
- Conduct digital ward rounds to assess patient medication profiles and identify potential medication-related issues.
- Assist pharmacists in follow up of recommendations for medication adjustments and interventions as required in hospitals.
- Participate actively in interdisciplinary discussions regarding patient care and medication management.
Safe and rational use of Medicine and Medical devices
- Perform comprehensive medication reviews for patients, identifying and resolving medication-related problems.
- Monitor patient responses to medications and make necessary interventions to ensure optimal therapeutic outcomes.
- Collaborate with the hospital teams to ensure timely resolution of medication-related problems.

Clinical Consultation
- Provide support to healthcare professionals by offering information on drug interactions and dosage adjustments.
- Provide evidence-based recommendations on medication therapy management, drug utilization, and formulary management.

- Maintain accurate and detailed patient records, documenting interventions, recommendations, and outcomes of medication reviews.
- Assist in generating reports summarizing medication-related interventions for internal and external stakeholders.

- Actively engage in continuous education and training to enhance clinical knowledge and pharmacy skills.
- Seek guidance and mentorship from digital clinical pharmacist leads to improve expertise in clinical pharmacy practice.
- Provide training and mentorship to pharmacists, therapeutic committees, share expertise and knowledge to enhance the team's overall capabilities.
SKILLS PROFILE
EDUCATION
Required
- B. Pharm degree
Preferred
- A qualification deemed to be equivalent or higher than the professional master’s degree in clinical pharmacy.
- Minimum of 2 years of clinical pharmacy experience, with a focus on medication therapy management and patient care.
WORK EXPERIENCE
Required
- Strong knowledge of evidence-based medicine, pharmacology, and pharmacotherapy guidelines.
- Excellent communication (written and verbal) and interpersonal skills, with the ability to collaborate effectively with interdisciplinary teams.
KNOWLEDGE
Required
- Proficiency in utilizing digital healthcare platforms and technologies like CareOn, SAP and IMT.
- Proficiency in Microsoft 365 applications like Excel and PowerPoint.
- Eagerness to learn, adapt, and grow within the field of clinical pharmacy.
PHYSICAL REQUIREMENTS

- Professional flexibility in working hours while supporting daily business operations which may include weekends and night shifts where applicable.
