Description
Support Long-Term Mobility and Independence at Home.
@Home Therapy is seeking a Physical Therapist to provide in-home rehabilitation services to adult patients in Manhattan. This role focuses on improving mobility, restoring functional movement, and helping patients maintain independence within their home environments. You’ll be working in a community-based setting where therapy is directly applied to real-life movement challenges inside the home.
What You’ll Be Doing:
Home Mobility Assessment:
- Evaluate balance, gait, strength, range of motion, and functional mobility in the home environment.
- Identify environmental and physical barriers that may affect safe mobility and independence.
Functional Rehabilitation Services:
- Develop and implement individualized treatment plans to improve or maintain mobility, strength, balance, and functional independence.
- Provide rehabilitation for patients recovering from illness, injury, surgery, or managing chronic conditions.
Real-World Movement Training:
- Train patients in safe transfers, ambulation, stair negotiation (when applicable), and household mobility.
- Educate patients and caregivers on fall prevention, safe movement strategies, and the use of assistive devices.
Ongoing Progress Monitoring:
- Monitor patient progress, reassess functional status, and modify treatment plans based on clinical findings.
- Collaborate with physicians and the interdisciplinary care team to support patient goals.
Documentation & Compliance:
- Complete timely documentation using the agency's electronic medical record (EMR) system.
- Maintain compliance with clinical, regulatory, and agency documentation standards.

Qualifications:
- Active New York State Physical Therapist license.
- Experience with adult rehabilitation preferred.
- Strong understanding of gait, balance, and functional movement training.

- Strong communication and clinical reasoning skills.
