Position Summary: Make a difference in the life of a young person as a Program Assistant in the Youth ACT (Assertive Community Treatment) program. Work directly with a team that helps children ages 10-21 avoid hospitalizations and out-of-home placements. The team will work to provide skills and support to meet the needs of youth and families to overcome challenges. The program assistant serves as a non-clinical staff that uses a range of administrative assistant skills to keep the team organized and focused on providing high-quality care.
Responsibilities:
- Maintaining accurate and up to date logs and case records in the agency Electronic Health Record (EHR)
- Serve as a frontline representative for Youth ACT participants, families, and supports, via phone, email, and in person communication
- Support team with care/case coordination
- Remain engaged and informed by attending daily team meetings and take notes of the meeting for the team
- Attend specialized training in the Youth ACT model
- Take initiative, work independently
- Support the team in needed tasks which could include, placing calls, scheduling appointments, syncing calendars, running errands, etc...
- Performing reception and other office duties that are needed
- Completing quality call backs to ensure services provided are consistent with Youth ACT model
Requirements
Education:
- High School Diploma required
- Associates Degree in related field preferred
Experience:
- 1 year of administrative assistant service
- Driver's License and ability to maintain insurability throughout employment required
