Position Summary
The Speech Language Pathologist (SLP) is responsible for evaluating, developing, and implementing individualized treatment plans for patients with speech, language, communication, cognitive, and swallowing disorders. The ideal candidate is compassionate, organized, and committed to helping patients achieve their highest level of function and independence.
Essential Duties & Responsibilities
Evaluate patients and develop individualized treatment plans based on clinical findings
Provide speech, language, cognitive, voice, and swallowing therapy services
Monitor patient progress and modify treatment plans as appropriate
Maintain accurate, timely, and compliant documentation in accordance with facility, state, and federal guidelines
Collaborate with physicians, nursing staff, therapists, and other interdisciplinary team members to coordinate patient care
Educate patients and family members regarding treatment goals, progress, and home strategies
Participate in care conferences, discharge planning, and interdisciplinary meetings
Ensure compliance with all applicable state, federal, and company policies and procedures
Qualifications
Graduate of an accredited Speech Language Pathology program
Current state Speech Language Pathologist license or eligibility to obtain licensure
Certificate of Clinical Competence (CCC-SLP) preferred
