Hourly Pay Range:

$57.79 - $79.76 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

• Position: Advanced Practice Provider (APN/PA) – Center for Cardiac Health - Heart Failure
• Location: Edward Hospital, Naperville, IL
• Full Time/Part Time: Full-Time (average of 40 hours/week)
• Hours: Monday – Friday between 7:00AM – 5:30PM. Call in the future may be required.
• Required Travel: Possible travel to Elmhurst Specialty Clinic may be required.
• Outpatient role

Job Description:

Exciting opportunity to Join Endeavor Health’s Center for Cardiac Health–Heart Failure team in an autonomous role delivering patient-centered care to high-acuity patients under the direction of a collaborating physician! Key responsibilities include:

• Obtaining comprehensive patient histories and performing physical examinations
• Ordering and interpreting diagnostic tests
• Developing differential diagnoses and prescribing evidence-based pharmacological and nonpharmacological treatments
• Managing acute and chronic conditions
• Providing ongoing education to patients and their families
• Coordinating care with physicians and interdisciplinary teams to optimize outcomes and support post-discharge follow-up and transitional care
• Supporting quality-improvement and patient-safety initiatives
• Contributing to the development and implementation of organizational and service-line goals
• Adhering to all Endeavor Health policies, procedures, and standards.

Qualifications:

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required

• Education: Completion of master’s level program in specialty area
• Certification: BLS/CPR certification required
• Experience: Prior APN/PA Cardiology experience required

Special Knowledge and Skills Required:

• APP experience in cardiology required

 

Preferred Qualifications: 

• Experience working with Heart Failure patients preferred
