Description

The Community Health Outreach Specialist plays an important role in improving the health and well-being of Mono County residents by connecting individuals and families with essential public health services and resources. Working under the direction of Public Health staff, this position provides case management, health education, program outreach, and client support while helping residents navigate available services. The role also maintains program records, collects required data, and assists with community education efforts. Success in this position requires strong communication skills, cultural sensitivity, compassion, and the ability to build trust with diverse populations across the County.

They communicate clearly, listen with empathy, and are comfortable educating individuals and groups on health-related topics in a culturally responsive and age-appropriate manner. They have experience with case management, customer service, public assistance, or community outreach and can confidently explain programs, determine eligibility, and connect residents with available resources.

The successful candidate is self-motivated, exercises sound judgment, maintains strict confidentiality, and is comfortable working independently while collaborating with healthcare professionals and community partners. Bilingual skills, particularly the ability to provide verbal and written translation services, are highly valued. Because the position serves residents throughout Mono County, the ideal candidate is flexible, dependable, and willing to travel to remote communities in varying weather conditions.


About the Department

The Public Health division of Mono County Health and Human Services is dedicated to protecting and improving the health of individuals, families, and communities through prevention, education, outreach, and access to quality public health services. The department provides a broad range of programs that promote healthy lifestyles, prevent disease, support maternal and child health, increase access to immunizations, and connect residents with vital health resources.

Mono County, Located in California’s Eastern Sierra

Mono County is a rural region with a dynamic community of 13,000 residents located in the eastern side of California’s Sierra Nevada range. Development is increasing in the area due to the consistent growth of the town of Mammoth Lakes. TRAINING AND EXPERIENCE:
Any combination of training and experience which would provide the required knowledge and abilities is qualifying. A typical way to obtain the required knowledge and abilities might be:
A minimum of two (2) years of experience that may include any of the following:
  • Working with people exhibiting strong communication skills both orally and in writing; 
  • Experience presenting information to others or educating others; 
  • Counseling and case management; 
  • Determining eligibility for a particular program.
Completion of college courses in public health, health promotion, education, or media related field is desirable.

TYPICAL PHYSICAL REQUIREMENTS:

  • Sit for extended periods; 
  • Frequently stand and walk; 
  • Manual dexterity and eye-hand coordination; 
  • Corrected hearing and vision to average range;
  • Verbal communication; 
  • Use of audio-visual equipment; 
  • Use of office equipment including computers, telephones, calculators, copiers, and FAX.

TYPICAL WORKING CONDITIONS:
  • Work is performed in clinics, office, and outdoor environments; 
  • Possible exposure to communicable diseases and blood borne pathogens; 
  • Contact with other staff and the public. 
  • Incumbents may be required to drive in remote areas of the County in all weather conditions.
