Western Geriatrics and Neurology, part of Brigade Health, is hiring compassionate Nurse Practitioners (NPs) and Physician Assistants (PAs) to provide high-quality mobile primary care to older adults in the comfort of their homes, skilled nursing facilities, assisted living, and senior communities.

What You’ll Do

• Provide mobile primary care visits to seniors
• Manage chronic and acute conditions
• Perform comprehensive assessments & care planning
• Collaborate with caregivers, facilities, and interdisciplinary teams
• Deliver compassionate, patient-centered care
• Maintain accurate EMR documentation

Qualifications

• Active NP or PA license
• 1+ years of primary care, geriatrics, home health, hospice, SNF, or senior care experience preferred
• Strong communication and clinical skills
• Valid driver’s license & reliable transportation
