Summary
Designs, provides, and evaluates planned diabetes education programs for patients and family members, physicians, community members, LVHN employees, and business and industry clients based on specific population needs and activities.

Job Duties

• Works in conjunction with diabetes program coordinator to perform DSME program education.
• Plans, provides, and evaluates diabetes education programs.
• Assesses the patient's readiness for change and identifies potential barriers to behavior change.
• Assists in development, planning, evaluation, and summary of departmental or multidisciplinary projects.
• Completes annual unit competencies as required.
• Coordinates with LVHN colleagues, physician practices, and community members to improve diabetes care delivery in region.
• Develops, with the patient, an individualized education plan that is based on the seven key behaviors for self-management of diabetes.
• Documents all educational interactions with clients on forms and provides feedback to referring providers.
• Develops and revises educational curriculums and materials with attention to program recognition requirements and national standards.
• Evaluates behavior change progress and change education plan to promote patient goal achievement and successful self-management.
• Obtains, interprets, and prioritizes information about individual patient and/or caregiver needs.
• Provides data to support billing by the support staff responsible for billing.

Minimum Qualifications

• Bachelor’s Degree in nursing or related field.
• 2 years experience in clinical patient education. and
• 5 years experience direct diabetes patient education.
• CDE - Certified Diabetes Educator NCBDE - State of Pennsylvania Upon Hire
• American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
• RD - Registered Dietitian - State of Pennsylvania Upon Hire or
• RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire

Preferred Qualifications

• Master’s Degree in nursing or related field.

Physical Demands
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
