This position will support McHenry and the surrounding areas.

As a Physical Therapist, you’ll contribute to our success in the following ways:

• Evaluate patients and develop individualized treatment plans. 
• Provide physical therapy services in the home setting. 
• Monitor patient progress and adjust care plans as needed. 
• Collaborate with physicians, caregivers, and the interdisciplinary care team. 
• Complete timely and accurate clinical documentation. 
• Supervise therapy assistants and support staff, as appropriate. 

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program 
• Current, unrestricted PT license valid for the state of work 
• Minimum one (1) year of experience in a healthcare facility
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations. 
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

The base salary for this position is $50 to $60 per hour, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
