At Concho Hearts Hospice, we're looking for a compassionate, dependable, and motivated Full-Time Hospice RN Case Manager to join our growing team serving Plano, Texas, and surrounding communities.

What You'll Do:
- Manage an assigned caseload of hospice patients and provide compassionate, individualized care.
- Perform comprehensive nursing assessments and routine patient visits.
- Develop, implement, and update individualized care plans.
- Provide pain management, symptom control, and comfort-focused nursing care.
- Monitor changes in patient conditions and communicate with physicians and interdisciplinary team members.
- Educate patients and families on medications, disease progression, symptom management, and end-of-life care.
- Coordinate patient care with physicians, LVNs, Hospice Aides, Social Workers, Chaplains, and other team members.
- Participate in interdisciplinary team (IDT) meetings and care planning.
- Ensure accurate and timely documentation in the electronic medical record (EMR).
- Communicate effectively with patients, families, caregivers, and the clinical team.
- Advocate for patients and families while providing emotional support throughout their hospice journey.
- Maintain compliance with hospice regulations, company policies, and clinical standards.
What We're Looking For:
- Active Texas Registered Nurse (RN) license or qualifying Nurse Licensure Compact (NLC) license.
- RN Case Management experience is a plus!
- Strong clinical assessment and critical-thinking skills.
- Knowledge of pain management, symptom control, and end-of-life care.
- Excellent communication, organization, and time-management skills.
- Ability to independently manage an assigned patient caseload.
- Compassionate, dependable, and committed to exceptional patient care.
- Ability to collaborate effectively with an interdisciplinary team.
- Current CPR certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- Ability to travel throughout Plano and surrounding communities.
