SUMMARY (Basic Purpose of the Job)
Under the direction of the Institute of Urban Care Manager, responsible for daily operations of program and grant management including interacting with community partners, oversight of budget, data collection, analysis and reporting. Leads project management for program specifically related to hospital based violence prevention program within the health system and out in the community. In coordination with leadership, develops, implements, evaluates and improves policies, programs and services consistent with organizational strategic mission. Coordinates and administrates intervention and follow-up services providing intensive case management and mentoring of the target population experiencing violence, as well as the field training of program staff, resource development and report writing.
MINIMUM REQUIREMENTS
Education:
High School Diploma or equivalent. Equivalent college coursework in social services field preferred.
Experience:
Three years case management experience required. Project management and data analytics experience preferred. Applicants with personal experience in overcoming violence/ violence-related injuries/criminal justice system are encouraged to apply.
Other Credentials:
AHA BLS. Non-Licensed must obtain before end of orientation period
Knowledge and Skills:
Demonstrated commitment to working with target population. Knowledge of urban violence issues. Punctual and extremely reliable a must. Highly organized and detail-oriented. Ability to perform assessments with inpatient and emergency room trauma patients. Must be able to present self and program in a professional manner. The expectation of this position requires full-time on-site work and flexibility to work some evenings and weekends.
Special Training:
Proficient in MS Office including PowerPoint, Outlook, Word. Knowledge of query tools and web-based applications and developing complex Excel spreadsheets.

ESSENTIAL FUNCTIONS
- Uses information technology in accessing, collecting, analyzing, maintaining and disseminating data and information.
- Evaluates the integrity, validity and reliability of data with the goal of analysis, trend identification, and gap resolution.
- Examines the feasibility (ex. fiscal, social, political, legal) and implications of policies, programs and services.
- Recommends, implements and evaluates policies, programs and services (ex. outputs, outcomes, processes, procedure and return on investment) in collaboration with Institute of Urban Care Manager.
- Provides intensive case management, mentoring and advocacy to target population involved in violence.
- Provides client referrals to community service providers.
- Maintains intensive follow-up contact with clients, family and service providers through frequent home visits and telephone contact.
- Documents consistently and accurately in written and computerized records all contacts with clients.
- Communicates in writing and verbally with linguistic and cultural proficiency. Writes all program reports to grant funders.
- Oversees monthly report data entry and report generation.
- Facilitates field training and supervises all newly hired Outreach Specialists.
- Facilitates communication, collaboration, program development and evaluation among partners with the goal of improving health in the community.
- Collaborates in community-based participatory research.
- Develops, justifies, and monitors program budgets and collaborates with Executive Director. Oversees continual updates of the Resource Guide.
- Maintains affiliation with service partners and researches/develops new relationships with community service providers to secure additional resources for program participants.
- Manages accounts payable requests.
- Manages staff schedules.
- Represents program at community meetings and participates in violence prevention efforts with other providers as assigned. Represents program to the media, public officials, community leaders, etc. on strategies to prevent violence and improve services to the target population.
- Coordinates weekly case management and monthly leadership meetings. Participates in weekly, monthly and quarterly staff meetings.
- Performs other duties and responsibilities as assigned.
