Key Responsibilities:
Lead, Registered Nurse (RN):
- Utilizes nursing knowledge and skills in the safe implantation of basic preventative, therapeutic, and rehabilitative nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes.
- Maintains current knowledge/certification/licensure.
- Demonstrates the skills and judgment necessary to implement medical plan of care, nursing interventions, and procedures necessary for the care of the patient.
- Perform delegated nursing functions using established procedures, policies, guidelines, and standards.
- Assists in data collection on the EMR. Documents pertinent observation and intervention on appropriate record.
- Contributes to the development of the patient/significant other plan of care based on the identified patient problems/needs. Reports needs of the patient and observations regarding condition to the provider.
- Carries out physician orders accurately, directly, and indirectly.
- Performs selected tasks related to patient care and the patient’s environment in a safe manner.
- Administers medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes.
- Maintain and coordinate communication among patient/significant other, staff, and physician as evidenced within medical record, client, and health care team feedback.
- Assists in assuring that effective communication exists with the patient/significant other as reflected in the plan of care.
- Work collaboratively with other members of the healthcare team in coordination of the patient’s care as evidenced by other department feedback.
- Assists patients, family members, visitors, or employees with concern and empathy; respects their confidentiality and privacy and communicate with them in a courteous, professional, and respectful manner.
- Answers and refers telephone calls or other inquiries to ensure accurate and timely communications as facilitated.
- Assists in the orientation/training of new Team Members and functions as a role model for current/new staff.
- Consistently works in a positive and cooperative manner with fellow Team Members, other Team Leads, and Nursing Supervisors.
- Takes direction and initiate actions (cross/additional training) that will allow the assumption of cross-functional duties to ensure seamless patient care.
- Demonstrates flexibility to perform duties wherever volume deems it necessary within the Clinic.
- Demonstrates sound judgment by taking appropriate actions regarding questionable findings or concerns.
- Ensures compliance with regulatory standards.
- Develop schedules for team members.
Lead, Licensed Practical Nurse (LPN):
- Perform medication reconciliation with patients.
- Advise and schedule appropriate follow-up appointments.
- Carries out physician orders accurately, directly, and indirectly.
- Request appropriate medical records.
- Maintain and coordinate communication among patient/significant other, staff, and physician as evidenced within medical records, client, and health care team feedback.
- Effectively communicate patients’ changing needs and plans of care among staff members.
- Report patient needs/programs and observations regarding patient’s condition to the physician.
- Work collaboratively with other members of the healthcare team in coordination of the patient’s care as evidenced by other department feedback.
- Participates in staff development programs.
- Attend scheduled staff meetings and is responsible for information discussed.
- Assists patients, family members, or other clients with concern and empathy; respects their confidentiality and privacy and communicate with them in a courteous and respectful manner.
- Answers and refers telephone calls or other inquires to ensure accurate and timely communications as facilitated.
- Assists in the orientation/training/mentoring of new Team Members.
- Consistently works in a positive and cooperative manner with fellow Team Members, other Team leads and nursing supervisors.
- Assists other Team Members in the performance of their assignments.
- Takes direction and initiate actions (cross/additional training) that will allow the assumption of cross-functional duties to ensure seamless patient care.
- Demonstrates flexibility to perform duties wherever volume deems it necessary within the Clinic.
- Demonstrates sound judgment by taking appropriate actions regarding questionable findings or concerns.
- Consistently evaluates work and determines if further steps are needed to meet client expectations.
- Ensures compliance with regulatory standards.
- Consistently demonstrates ability to respond to changing situations in a flexible manner in order to meet current needs, such as reprioritizing work as necessary.
- Minimizes non-productive time and fills slow periods with activities that enables preparation to meet the future needs of the Clinic (education, organizing, housekeeping, assisting others.)
- Organizes job functions and work area to be able to effectively complete varied assignments within established time frames.
- Consistently demonstrates ability to take the initiative to make decisions/choices without direct supervision.

Requirements
Lead, Registered Nurse (RN):
Required Qualifications:
- Graduate of an accredited school of nursing
- One year of relevant work experience.
- Current, unrestricted MO RN license
- Current Basic Life Support (BLS) required, must obtain within first 90 days of employment.
- IV certified
Lead, Licensed Practical Nurse (LPN):
Required Qualifications:
- Graduation from LPN program and current, unrestricted MO LPN license.
- Minimum of two years’ clinical experience required
- Current Basic Life Support (BLS) required, must obtain within first 90 days of employment.
Preferred Qualifications:
- Experience in ambulatory clinical setting preferred
- Experience with electronic medical records preferred
