Physical Therapist
San Jacinto Valley Post Acute
Join our dedicated team and make a meaningful difference in the lives of our residents every day. We are seeking a compassionate and skilled Physical Therapist to provide exceptional rehabilitative care and help patients achieve their highest level of function and independence.
Position Summary
The Physical Therapist is responsible for evaluating, developing, and implementing individualized treatment plans to improve mobility, reduce pain, restore function, and enhance quality of life. This role collaborates closely with an interdisciplinary team to deliver outstanding patient-centered care.
Essential Responsibilities
- Evaluate residents and develop individualized treatment plans based on clinical findings and goals.
- Provide therapeutic interventions to improve strength, mobility, balance, endurance, and functional independence.
- Monitor patient progress and modify treatment plans as appropriate.
- Collaborate with physicians, nursing staff, therapy team members, residents, and families to ensure coordinated care.
- Educate patients and caregivers on treatment plans, exercises, and strategies to support recovery and long-term wellness.
- Maintain accurate and timely documentation in accordance with regulatory and facility requirements.
- Ensure compliance with all applicable professional, ethical, and safety standards.
Qualifications
Education & Licensure
- Graduate of an accredited Physical Therapy program.
- Current California Physical Therapist license, or eligibility for California licensure.
- Current CPR certification preferred.
Experience
- Previous experience in skilled nursing, rehabilitation, outpatient, or post-acute care settings is preferred.
- New graduates are encouraged to apply.

Compensation & Benefits
- Pay Range: $49.00 - $55.00 per hour DOE (Depending on Experience)
