RN Case Manager (RNCM) + Recertification Nurse - Home Health Agency (Part-Time: Weekdays, Daytime | 1-2 days/wk) | Brown County, OH - SE OH

JOB TITLE: Home Health RN Case Manager & Recertification Nurse (RNCM)

• This position will be hired as Part-Time only:
  • Requires at least 4 weekly visits, plus additional visits for recertifications, etc., as needed.
  • Requires commitment of 1-2 days per week, and a travel radius of up to 1 hour, with exact hours varying week to week.
    • Average Low end: 4 hours + travel
    • Typical: 4-8 hours + travel
    • Average High end: 8-15 hours (or more) + travel 
  • Additional Part-Time hours may also available with a 2-hour travel radius as well as willingness to provide coverage for Skilled Nursing visits (if desired and willing; not required).
  • Full-Time may be available in the future but not at this time and is not guaranteed.
• Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall.
• Requires back-up and occasional Skilled Nursing visits and services including med sets, dressing changes, etc.
• At least 1 year of experience is required in: Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
• At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent); 2-5 years' experience is preferred.
• Desirable but not required:
• Nurse Supervision experience
• People management
• Peer/staff education and development
• Success or willingness to build relationships with client payors, case mgr / entity relationships (i.e., AAA, DoDD County Board, VA, etc.) in SW Ohio and our current + prospective employees in these communities.

SERVICE AREA: throughout Brown County and surrounding areas of up to 60-minute radius in Southern Ohio.

• Primary / Required service areas include:
• Georgetown, Mount Orab, Sardinia + up to 60-minute travel radius
• including adjacent Adams, Highland, Clinton, and Clermont Counties within radius.
• HIGHLY DESIRED (but not required): Additional case/hours are available for those willing to travel up to 2-hour travel (not required), to clients in/around:
  • Adams, Highland, Clinton, and Clermont Counties beyond 60-minute radius.
  • Occasional/rare coverage in Scioto, Pike, Jackson, Ross, Fayette, Hamilton, Warren, Clinton, Montgomery, and Greene counties.
• Qualified Applicants may reside in or near any part of the service areas so long as travel requirements are fulfilled.
• NO Overnight travel required.
• NO Weekends required.
• NO On-Call Shifts required.

EMPLOYMENT & SCHEDULE TYPE: Long-term employment for Part-Time RNCM/Recert Nurse

• This role requires a 1-2 days/week commitment to perform at least 4 visits (weekly) + additional recertifications/visits as needed.
• 1-2 day/wk Availability is required between Monday - Friday, during daytime hours (8am - 5pm). 
  • Occasional/rare evening availability (scheduled in advance) may be offered / not required.
• Candidates seeking long-term employment are preferred, however short-term or temporary employment will be considered.
  • YES: We are happy to work around any primary employer schedules so long as availability requirements are met.
• RN and Part-Time Nurses are paid Hourly, via weekly direct deposit on Fridays.
• Round-Trip/Door-to-Door mileage is reimbursed.
• Requires travel throughout pre-determined service area (minimum 1-hour travel radius); Willingness to travel up to approx. 2-hour daily radius) highly desirable.

Compensation:

• Hourly Wage (+ Mileage) starts between $35 - $40 per hour, based on minimum 1-hour travel radius (required) and up to 2-hour / total area travel coverage (highly desired).

Job Summary As an RNCM/Recertification Nurse, you will be instrumental in coordinating, re-evaluating and confirming the medical necessity of continued home health care services for our patients, and providing patient care and ensuring optimal outcomes.

• With core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
• You will submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.
• You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Responsibilities/Requirements of an RN Case Manager / Recertification Nurse include:

• RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
• RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
• RN to Document/Communicate/Coordinate patient status/change in condition to care team.
• RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
• Willing to travel surrounding counties.
• Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.

Reports to: Regional Program Administrator, or Clinical Administrator/Director of Nursing

 

Qualifications

• Graduate of an accredited School of Nursing.
• Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
• Minimum of 1 year of experience in home health agency or equivalent setting.
• CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
• Experience with OASIS and NON-OASIS assessments is also required.
• Strong background in nursing, patient care, and confidentiality.
• Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid)
• Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
• Familiarity with HIPAA regulations and medical terminology.
• Willingness and ability to perform required physical demands (see below)
• Consent + Ability to pass a State & Federal background check (including fingerprints)
• 18yo + active Driver's License / State ID + US Work Authorization
• Reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients (up to at least 2 hours is required).
