Responsibilities:
- Coordinates and facilitates collaborative client care with families and Home visitation staff.
- Manages and facilities evaluations, assessments and home visitation services utilizing an approved curriculum.
- Admits families (prenatal to children up to age three) to the Home visitation program.
- Connects family with formal and informal community networks.
- Fosters multidisciplinary collaboration and decision making with the clients and their families.
- Maintains a working knowledge of a current list of available resources for referrals. Knowledge of infant/child development.
- Assures compliance with the contractual standards and requirements.
- Data collection and documentation according to contractual requirements.
- Maintains an active case load to have at least 40% of time with direct family contact.
- Attends all required contractual trainings
- Performs other functions as required.
Qualifications:
- Knowledge of pre-conception to Early childhood development.
- Function as member of the Early childhood network Infant Mental Health endorsement within 3 years of hire.
- Bilingual preferred.
