Description
- Provide one-on-one physical therapy treatments in patients' homes.
- Carry out individualized treatment plans established by the supervising Physical Therapist.
- Help patients improve mobility, strength, balance, endurance, safety, and overall function.
- Provide gait training, transfer training, therapeutic exercise, and fall-prevention interventions.
- Educate patients and caregivers on exercises, mobility strategies, and home safety.
- Monitor the patient's progress and communicate any changes to the supervising PT.
- Complete accurate and timely clinical documentation.
- Manage your daily schedule and caseload with support from our administrative team.
- $58,000–$75,000 annually.

- Home health or geriatric experience is preferred but not required.
