Description
Working hours are from 8 am until 5 pm Monday through Friday. This includes a one-hour lunch each day that falls between the hours of 11 am and 2 pm.The position requires the following knowledge, skills and abilities:
Knowledge of criminal justice and mental health/substance use systems and populations.
Thorough knowledge of community resources. Knowledge of Microsoft Office products, especially Excel, Outlook, and Word Documents.
Ability to accept referrals for court, gather clinical information, interpret information, and relay information to the court treatment team.
Ability to collect data as it pertains to coordination efforts and program success.
Ability to organize and facilitate treatment team and advisory meetings and maintain meeting notes.
Ability to establish and maintain relationships with participants, provider agencies, and community stakeholders.
Graduation from a four-year college or university with a degree in Psychology, Human Services, Social Work or closely related degree; and
Five or more years of Mental Health, Substance Use practices and treatment, case management experience and experience working with legal/criminal justice system (particularly the court system and with mental health courts).
Valid driver's license is required to attend offsite meetings and trainings.Essential duties and functions include but are not limited to:
Accept referrals from attorneys and the community for Forsyth County Mental Health Court (FCMHC), document actions taken after referrals are received, and consistently adheres to established protocols and timelines.
Initiates gathering of documents for referrals including, but not limited to, obtaining signed release of information, relevant treatment history, available psychiatric evaluations and relevant court history.
Conducts review to include treatment history and court involvement and communicates this information to FCMHC Treatment team.
Provides follow-up and support calls to participants and providers as needed and as assigned.
Has primary responsibility for the collection and reporting of data regarding service coordination efforts and treatment compliance. Maintains tracking data including input from participants, providers and others involved in treatment process.
Assists with and participates in treatment team meetings and provides any pertinent information to the team that is needed in order to develop and implement recommendations related to entry into FCMHC, rewards/sanctions and aftercare.
Establishes and maintains collaborative relationships with provider agencies and community stakeholders including the court system, law enforcement, magistrates, homeless shelters, community agencies and residential facilities as applicable.
Documents all care including telephonic and other forms of communication in the electronic member record (CureMD) or by progress note.
Expected to assist with any tasks associated with the Stepping Up Program as needed and as directed by program supervisor.
