Social Worker III

Vacancy is in the Epidemiology Division of the Health and Human Services Department.  Under general supervision, takes the lead in performing social services work. Performs increasingly difficult and complex social casework services; Interviews clients; collects information; assesses needs; provides assistance or makes referrals to other agencies or organizations. Delivers extensive social services to People Who Use Drugs (PWUD) and their families. Supervises and reviews the activities of social workers of a lower class. Responsible for coordinating and providing relocation assistance to Hartford Residents, including temporary, emergency housing. Provides daily case management and referrals to clients. Develops and maintains detailed case records of client services. Attends case conferences and staff meetings and interprets programs to clients, other agencies and interested individuals. Writes weekly reports for case progress for informed decision making. Collaborates with housing and community partners on relocation cases. Prepares and maintains a variety of statistical records and reports. Works directly with Hartford residents affected by substance use disorder, connecting them to care through referrals. Assists in the development of clinical, community, and public safety trainings. 

This is a HMEA position.  The hours of work for this position are 40 per week and the above salary includes 5% in lieu of overtime.

• Plan, organize and supervise the work activities of subordinates.
• Interview clients, assess their needs and recommend appropriate action.  
• Prepare detailed casework records for client.
• Complete a variety of forms and documents. 
• Provide extensive counseling on a variety of subjects.
• Communicate effectively, orally and in writing.
• Establish and maintain effective working relationships with clients and outside agencies.
