Help transform the way care is delivered across our ambulatory practices.

 

ChristianaCare is seeking a Practice Transformation Liaison to partner with primary care and ambulatory practices to improve workflows, strengthen quality outcomes and support the delivery of value-based care.

 

We’re looking for a relationship builder, problem solver and change partner with strong customer service skills and a solid understanding of ambulatory or primary care operations.

 

This is a highly collaborative, relationship-driven role for someone who understands how physician practices operate and recognizes that no two practices are exactly alike. You’ll work directly with providers, nurses, practice leaders and front-office teams to understand their needs, identify opportunities, and help implement practical solutions that improve both practice performance and the patient experience.

 

What You’ll Do

  • Build trusted partnerships with providers, practice leaders, and clinical and administrative teams across our ambulatory network.
  • Assess and improve practice operations by evaluating workflows, identifying barriers and implementing solutions that improve efficiency, quality and the patient experience.
  • Advance value-based care by helping practices improve preventive care, chronic disease management, quality outcomes, and closure of care gaps.
  • Lead and support change by educating practice teams, using performance data to identify opportunities, and collaborating across Population Health, Quality, Data Analytics, IT and other partners.

 

Qualifications

  • Bachelor’s degree in a clinical discipline, operations, business administration, healthcare management, statistics or related field required; master’s preferred.
  • Minimum 5 years of relevant experience, including 2 years in patient safety, quality or performance improvement. Equivalent ambulatory-focused experience may be considered in lieu of a bachelor’s degree.
  • Knowledge of ambulatory/clinical practice operations, electronic medical records and medical terminology.
  • Strong communication, customer service, organizational, project management and problem-solving skills.
  • Ability to work independently, manage multiple priorities and collaborate effectively across teams.
  • Experience with quality improvement, workflow analysis, change management or population health preferred.
  • Travel throughout ChristianaCare and participating clinical practices is required.

Annual Compensation Range $87,588.80 - $140,129.60

This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
